RESUMO

El espacio parafaríngeo es una región donde puede surgir un amplio espectro de tumores, que son poco frecuentes y comprenden solo el 0,5 al 1,5% de las neoplasias de cabeza y cuello. Generalmente, son hallazgos y suelen presentarse como masas asintomáticas. Incluye neoplasias primarias, extensión directa de regiones adyacentes y enfermedad metastásica. La literatura describe múltiples patologías e histologías que pueden surgir de esta zona, entre ellas, tenemos los tumores fibrosos solitarios, que son un espectro de neoplasias mesenquimatosas fibroblásticas de histogénesis incierta, que generalmente involucran la pleura. Son lesiones muy poco frecuentes y su localización parafaríngea es excepcional, siendo su primer reporte en la literatura en 1993. Se presenta el caso de un tumor fibroso solitario del espacio parafaríngeo evaluado en nuestro centro, con su enfrentamiento diagnóstico, resolución quirúrgica y seguimiento respectivo, con el objetivo de presentar esta patología poco frecuente a nuestros colegas y contribuir al conocimiento médico.


The parapharyngeal space is a region where a wide spectrum of tumors can arise, which are rare and comprise only 0.5% to 1.5% of head and neck neoplasms. They are generally findings and usually present as asymptomatic masses. Includes primary neoplasms, direct extension from adjacent regions, and metastatic disease. The literature describes multiple pathologies and histologies that can arise from this area, among them, we have solitary fibrous tumors, which are a spectrum of fibroblastic mesenchymal neoplasms of uncertain histogenesis, which are generally the pleura. It is a very rare lesion and its parapharyngeal location is exceptional, being its first report in the literature in 1993. We present the case of a solitary fibrous tumor of the parapharyngeal space evaluated in our center, with its diagnos- tic confrontation, surgical resolution and respective follow-up, with the aim of presenting this rare pathology to our colleagues and contributing to medical knowledge.

RESUMO

El tumor de Pott es una entidad rara, definida como un absceso subperióstico asociado a osteomielitis del hueso frontal. Suele presentarse como complicación de sinusitis aguda o crónica del seno frontal, y se describe con mayor incidencia en población pediátrica, siendo una complicación grave por su alta morbimortalidad. Se presenta a un paciente pediátrico con tumor de Pott y absceso cerebral posterior a una sinusitis aguda de foco odontogénico, en la que los pilares de tratamiento son drenaje quirúrgico precoz y antibioticoterapia prolongada. Se describe la clínica, manejo médico-quirúrgico y seguimiento posterior, ya que, en ausencia de antecedentes, se debe buscar dirigida- mente enfermedad periodontal y realizar un estudio de inmunodeficiencia primaria.


Pott's tumor is a rare entity, defined as a subperiosteal abscess associated with osteomyelitis of the frontal bone. It usually presents as a complication of acute or chronic sinusitis of the frontal sinus and it is described with a higher incidence in the pediatric population, being a serious complication due to its high morbidity and mortality. We present a pediatric patient with Pott's tumor and brain abscess after acute sinusitis of odontogenic focus, in which the pillars of treatment are an early surgical drainage and prolonged antibiotic therapy. The clinic, medical-surgical management and subsequent follow-up are described, since in the absence of antecedents, periodontal disease should be sought directly and a study of primary immunodeficiency performed.

El manejo del sangrado en CEF es fundamental, ya que aumenta la tasa de complicaciones. El principal objetivo del manejo perioperatorio es lograr una buena hemostasia, es por esto que se realizó una revisión de las principales recomendaciones de la literatura internacional, que incluye medidas preoperatorias, intraoperatorias y postoperatorias.


Bleeding is an important factor in FESS since it increases the rate of complications. The main objective of perioperative management is to achieve good hemostasis. This is a review of the main recommendations of the international literature, that includes preoperative, intraoperative and postoperative measures.

Los tumores malignos de cavidad nasal y cavidades paranasales son poco frecuentes y sus síntomas de presentación suelen ser unilaterales e inespecíficos; éstos incluyen: epistaxis, rinorrea y obstrucción nasal. Encontramos una amplia variedad histológica de tumores, entre ellos tenemos el angiosarcoma nasal, que es considerado una rareza y el diagnóstico definitivo lo da la confirmación anatomopatológica complementado con estudios inmunohistoquímicos. Su modalidad de tratamiento incluye múltiples opciones, pero la cirugía radical temprana con márgenes negativos asociado a radioterapia adyuvante, ofrece el mejor pronóstico. Se presenta a un paciente adulto masculino con un angiosarcoma nasal izquierdo que fue detectado de forma precoz y se manejó con cirugía radical y radioterapia.


Malignant tumors of the nasal cavity and paranasal cavities are rare, while presenting symptoms are usually unilateral and nonspecific like, epistaxis, rhinorrhea, and nasal obstruction. There are many histological varieties of tumors, among them we have nasal angiosarcoma, which is considered a rarity pathology. The definitive diagnosis is given by biopsy confirmation complemented with immunohistochemical studies. Its treatment modality includes multiple options, but early radical surgery with negative margins associated with adjuvant radiotherapy offers the best prognosis. We present a male adult patient with a left nasal angiosarcoma that was detected early and managed with radical surgery and radiotherapy.

The use of photothermal techniques has become of special importance due to their versatile application in the thermal characterization of materials. Therefore, the thermal lens technique in the mismatched dual-beam mode is an alternative, sensitive and non-evasive tool that was used in this research to determine the thermal diffusivity of Moringa oleifera. The dual arrangement of the thermal lens technique is based on the use of an Ar+Xe excitation laser (422 nm) and a He-Ne laser (632 nm) test laser. Moringa solutions were prepared by green synthesis with different concentrations ranging from 1.56 mg·mL-1, 3.12 mg·mL-1, 6.25 mg·mL-1 to 12.50 mg·mL-1. Different optical techniques (UV-vis, FTIR, XPS and EDS) were used to characterize the Moringa leaf powders. Results showed that the increase of thermal diffusivity could be related to the presence of functional groups and metallic elements in Moringa elemental composition. In this work, it was found that the thermal diffusivity of Moringa increases with increasing concentration. This study will be useful for application in heat transport and drug release.

RESUMEN La rinosinusitis bacteriana aguda es la infección de la mucosa nasal y de senos paranasales, que se manifiesta con congestión nasal, rinorrea purulenta y fiebre, de curso generalmente autolimitado. La trombosis del seno cavernoso (TSC) es una complicación poco frecuente, que se asocia a compromiso del seno esfenoidal y etmoidal, manifestándose clínicamente con cefalea, fiebre, alteración de pares craneanos y síntomas oculares. Ante su sospecha el estudio imagenológico es fundamental. En estos exámenes se debe buscar un defecto de llene en el seno cavernoso comprometido. El tratamiento debe iniciarse de forma precoz y contempla: antibioticoterapia, drenaje quirúrgico del seno afectado y anticoagulación. Se presenta a un paciente pediátrico masculino con TSC derecha posterior a una rinosinusitis esfenoidal no tratada, que respondió favorablemente al tratamiento. Se describe la clínica y manejo de esta patología, que presenta alta morbimortalidad, y que puede disminuirse considerablemente con un tratamiento precoz y agresivo.


ABSTRACT Acute bacterial rhinosinusitis is the infection of the nasal and paranasal mucous membrane, with symptoms such as nasal congestion, purulent rhinorrhea and fever, usually self-limiting. Cavernous sinus thrombosis (CST) is a rare complication, which is associated with sphenoid and ethmoid sinus involvement, clinically manifest with headache, fever, cranial nerve involvement and ocular symptoms. When suspected, imaging studies will confirm the clinical impression. They will show abnormal flow parameters in the compromised cavernous sinus. The treatment has to start as soon as possible and consists in broad spectrum intravenous antibiotics, surgical drainage of the site of the infection and anticoagulation. We present a patient with a right CST secondary to a non-treated sphenoidal sinusitis, that responded favorably to treatment. The clinical and management of this pathology is described, which presents a high morbidity and mortality that can be reduced considerably with an early and aggressive treatment.

RESUMEN La orbitopatía tiroidea es una enfermedad autoinmune, en la que una reacción inflamatoria genera aumento de la presión orbitaria con protrusión de su contenido. A menudo es autolimitada y sus síntomas más frecuentes son retracción palpebral, exoftalmo y diplopía. Existen casos severos con compromiso de la agudeza visual por compresión del nervio óptico. El diagnóstico es clínico, pero debe complementarse con una tomografía computarizada. Su tratamiento depende de la gravedad y actividad de la enfermedad, siendo los procedimientos quirúrgicos, como la descompresión orbitaria, de elección en exoftalmo y neuropatía óptica compresiva. El pilar de tratamiento en la orbitopatía tiroidea severa es la cirugía descompresiva. Se han descrito múltiples técnicas, pero con limitaciones. La descompresión endoscópica transnasal, es considerada actualmente el procedimiento de elección, ya que permite una buena visualización de la pared medial, con resultados comparables y menores complicaciones, respecto a métodos tradicionales. Describimos un caso de oftalmopatía tiroidea severa, con exoftalmo, diplopía y disminución de la agudeza visual, en la que se realizó una descompresión endoscópica con muy buenos resultados.


ABSTRACT Thyroid orbitopathy is an autoimmune disease in which an inflammatory reaction generates increased orbital pressure with protrusion of its contents. It is often self-limiting and its most frequent symptoms are eyelid retraction, exophthalmos and diplopia. There are severe cases with compromised visual acuity due to compression of the optic nerve. The diagnosis is clinical, but must be complemented with a computed tomography scan. Its treatment depends on the severity and activity of the disease and the surgicals procedures such as orbital decompression is the best choice in exophthalmos and compressive optic neuropathy. The treatment in severe thyroid orbitopathy is decompressive surgery. Multiple techniques have been described, but with limitations. The transnasal endoscopic decompression is currently considered the gold standard, since it allows a good visualization of the medial wall with comparable results and less complications, compared to traditional methods. We present a case of severe thyroid ophthalmopathy, with exophthalmos, diplopia and decreased visual acuity, in which a transnasal endoscopic decompression was performed with very good outcomes.

The antibacterial property of thyme essential oil due to different volatile compounds, has been well documented in the literature. To overcome the high volatility of essential oil components, encapsulation has emerged as a new alternative. In this work, chitosan and thyme essential oil-loaded chitosan nanoparticles (TEO-CSNPs) and nanocapsules (TEO-CSNCs) were prepared by nanoprecipitation and nanoencapsulation, respectively. The morphology, encapsulation efficiency, release kinetics, and inhibitory activity were evaluated. Average size of nanocapsules (9.1±1.6nm) was slightly higher than nanoparticles (6.4±0.5nm). The percentage encapsulation of thymol and carvacrol, more than 68%, was similar for nanoparticles and nanocapsules. However, thymol and carvacrol release time from TEO-CSNPs was faster compared to TEO-CSNCs. The release kinetics data were fitted to three analytical kinetic models with no statistical differences among them. The inhibitory activity was higher for nanoparticles than for nanocapsules when tested against six foodborne bacteria. The inhibitory effect of TEO-CSNPs was the highest against Staphylococcus aureus (inhibition halo 4.3cm) and for TEO-CSNCs it was against Bacillus cereus (inhibition halo 1.9cm).

INTRODUCTION: Media are main allies to encourage donation and transplantation. We sought to quantify the presence in the media of the topic "Donation and transplantation (D/Tx)" during the period 2005 to 2007 and to determine the relationship between the volume of information issued by the media and indexes related to the topic. METHODS: We conducted a systematic recording of the news on the topic of D/Tx in print, radio, and television during 2005 to 2007. RESULTS: There was an increased presence in the media by 100% in 2005 and 2006, and by 200% in 2007. The presence in print media was greater than radio (249 +/- 31.5 vs 131 +/- 5.49; P = .05) or television (249 +/- 31.5 vs 92 +/- 7.12; P = .01). We observed a positive correlation between the total number of contacts versus the number of registered volunteer donors (R(2) = 0.9713) and number of contacts versus total number of transplants from cadaveric donors (CTX; R(2) = 0905). The index CTX per million population increased by 30% (2006 vs 2005) and 100% (2007 vs 2005). The percent of family refusals fell from 41% in 2005 to 34% in 2006 and to 31% in 2007. The rate of donors per million population grew steadily from 1.8 in 2005 to 2.3 in 2006, and to 3.6 in 2007. CONCLUSION: The sustained increase in D/Tx briefing as a strategy of Sistema de Procura de Organos y Tejidos has helped to foster a positive perception that is reflected in the increased rate of donation and transplantation in Venezuela.

El presente estudio fue realizado con el objeto de evaluar la funcion diastolica del ventriculo izquierdo en pacientes chagasicos asintomaticos. Se analizaron con este objetivo las ondas E de llenado rapido protodiastolico y la onda A de llenado lento telediastolico de contraccion atrial de los flujos sanguineos transmitrales por el metodo Doppler pulsado ecocardiografico en 10 pacientes (Grupo I) con serologia positiva para enfermedad de chagas, asintomaticos sin evidencia de enfermedad cardiaca 10 pacientes (Grupo II) con enfermedad cardiaca chagasica; 10 pacientes (Grupo III) con hipertrofia de ventriculo izquierdo y 10 sujetos (Grupo IV) control normales. La velocidad de la onda E protodiastolica se encoentro disminuida en relacion a la velocidad de la onda A en forma significativa (P<0.01) en el grupo chagasicos asintomaticos en relacion al grupo de normales; la disminucion de la onda E fue mas significativa (P<0.01). La velocidad de desaceleracion de la paciente de la onda E esta disminuida en forma significativa en el grupo II, esta disminucion no es significante (P<0.05) en los grupos I y III. Estos hallazgos demuestran que existe alteracion de la funcion diastolica de llenado ventricular izquierdo en pacientes chagasicos asintomaticos como evidencia de dano miocardico inicial, estas alteraciones son mas manifiestas en enfermos cardiacos chagasicos en los que ya hay disfunsion sistolica. Se considera que el Doppler pulsado ecocardiografico es un metodo no invasivo de facil acceso y de gran utilidad en la deteccion precoz de cardiomiopatia chagasica.

RESUMO

The work carried out in the Cardiocenter, in Villa Clara, during the period comprised from October 1986 to October 1987, is related in this paper. Statistical data were taken from the Service of Cardiovascular Surgery, expressing them in tables with analysis given in percentage. Opinions of the working team, which allowed us to research Nursing Care during the transoperative period, as well as some considerations that must be fulfilled in the operating rooms of this type of unit, were collected and enriched such aspects with bibliographies on the theme. Results obtained and the most frequent complications are stated.
